When you book a consultation, Calendly collects your name, email address, appointment date and time, and timezone directly. This data is governed by Calendly's own Privacy Policy at calendly.com/privacy. Earlens Corp receives a copy of your booking confirmation and contact details to facilitate the consultation.

Health-Related Information

The consumer contact form asks you to rate your hearing on a scale of 1 to 10 and indicate your timeline for seeking help. This is health-adjacent information. Earlens Corp does not sell it, does not use it for advertising targeting or profiling, and does not share it with advertising or analytics platforms. You may use an authorized agent with written proof of authorization.
